Biannual Reports

Grantees may visit www.samhsa-gpra.samhsa.gov to access the biannual report guidelines and template. Once the site is accessed, the grantee should click on CSAT-GPRA, then select Go. Click on the Services Info, and select Biannual Reports from the drop-down box.

Carry-Over Requests

Grantees should speak with their assigned government project officer to inquire about the amount of carry-over funds available.

Carry-over requests should be submitted to the Grants Management Officer at the Division of Grants Management.

Financial Status Reports

Fiscal and Program Progress Reports Financial Status Reports (FSR) are available online at: http://www.samhsa.gov/Grants/management.aspx. The FSR must be submitted to the Grants Management Branch (DGM) on an annual basis. The FSR is submitted on Standard Form (SF) 269 long form as the report of expenditures documenting the financial status of the award, according to the official accounting records of the grantee organization.
